A paper version of this form is also available from the reception desk at any of our 5 sites.

If possible, please bring a form of ID with you when you first visit the practice. If you do not have any ID then you are still welcome to register.

If you do not have a permanent home address then please state this on the registration form - this will not prevent you from being able to register.‍

Out of Area Registrations

If you do not live in our catchment are but still wish to be registered with us, you can request to be registered as an Out of Area patient.

Please note that these requests will be assessed on an individual basis, as there will be times where it is not clinically appropriate, practical or safe for a patient to be registered with us.

Also, if your health circumstances change, you could be advised to register with a practice closer to your home.

Registering with a GP surgery outside of your local area affects the NHS services you can access, including: 

• home visits and urgent care from your GP, including out-of-hours care
• community services like physiotherapy, midwife appointments and district nursing care

It is important that you consider this before registering, if you are out of area.

About Amicus Health

Amicus health has 5 sites which provide NHS GP serviced to patients in Mid Devon. These are

• Claire House Surgery in Tiverton
• Bampton Surgery
• Witheridge Medical Centre
• Morchard Bishop Surgery
• Cheriton Fitzpaine Surgery

We also provide remote consultations via telephone or videocall. We provide high quality personalised care and all patients who are registered with us have their own named GP.

If your own GP is not available and you need to speak to a doctor, you can book an appointment with any of the other GPs. There are currently 24 doctors working at Amicus Health.

We are committed to providing excellent locally-based care to our patients. We have a specialist surgical and dermatological service in Tiverton. We also run regular diabetes, contraception and joint injection clinics for our patients.
